The I-90 form from the U.S. Citizenship and Immigration Services is an application to replace a Permanent Resident Card, also called a green card. It can be completed by mail or online. You are eligible to file online if you are updating a card, replacing a card that is lost, destroyed or expired.

Go to the U.S. Citizenship and Immigration Services website and click on the "Forms" link. Click on "Green Card Based Forms" and select "Application to Replace Permanent Resident Card." Click on the link "Download Form I-90" to download the form. Type your answers in the form or complete it using black ink.

Fill out Part 1 of the form with personal and contact information. Indicate your immigration status and the reason you are filing the application in Part 2. Check only one box for your status and one for the reason for filing. Continue completing the form with processing information, disabilities and impairments and your signature. If someone else is filling out the form for you, have her sign as well.

Include a copy of your prior permanent resident card or other proof of identity as well as any supporting documentation. The fee to file is $365 plus an $85 biometric fee, for a total fee of $450. Make your check payable to U.S. Department of Homeland Security.

Mail the completed form, supporting documentation and filing fee to USCIS; P.O. Box 21262, Phoenix, Arizona 85036 for normal U.S. mail. If you use U.S.P.S. Express Mail, send the completed documents to USCIS; Attention: I-90; 1820 E. Skyharbor, Circle S, Floor 1 Ste. 100, Phoenix, Arizona 85034. When your form is received, USCIS will contact you by mail and tell you when to visit your local USCIS Application Support Center for your biometrics appointment.

Click on "Log in to e-filing" listed under Online Services on the right side of the I-90 Form page. Click "I am a new user" and check the Privacy Statement box. Click "Next."

Enter your personal information and create a user ID and password. Click "Submit." Complete Form I-90. The website will then show you a checklist with the required evidence and supporting documentation that you must mail in.

Mail all supporting evidence and documentation to the Nebraska Service Center, Attn: E-Filed I-90 Application; P.O. Box 87090, Lincoln, Nebraska 68502-7090. Your form will not be processed until all materials have been received.

Wait to hear from USCIS for the scheduling of your biometrics appointment. It may take several weeks. Bring your biometrics notice and photo identification such as your passport, driver's license or a state or military photo ID with you to the appointment.


Tips & Warnings


If you answer none on a question, write "NONE" and do not leave any question blank. If you want to be notified by email or text message when your form has been accepted, complete Form G-1145, E-Notification of Application/Petition Acceptance as well. Download it from the Form I-90 page and attach it to the first page of your application.
