Visit the USCIS homepage to download Form N-565, Application for Replacement Naturalization/Citizenship Document.

Download, complete and then print out Form N-565. Pay the processing fee for Form N-565. As of 2009, the fee is USD $380. To pay the fee, according to USCIS, make the check or money order payable to U.S.Department of Homeland Security.

Mail your Form N-565 and processing fee to the USCIS Nebraska Service Center or the USCIS Texas Service Center, depending on your state of residence. .

Wait to hear from USCIS about the status of your request. Depending on USCIS workload, it may take several weeks before you hear back from USCIS.


According to USCIS, it reserves the right to request more information or evidence and it may request that you appear at a USCIS office for an interview.
